At Acres of Diamonds, we are champions for moms and kids moving out of homelessness. We are a relational Christian ministry dedicated to helping moms rewrite their stories for lasting change for themselves and generations to come.
Our work is to come alongside each mom and her children and help them heal and grow. Our non-traditional community-based housing program includes counseling, case management, relationship skills development, employment support, parenting classes, financial management skill development, and much more. We work closely with each family to develop a program customized to their needs to maximize their success.
POSITION SUMMARY:
The Case Manager II primarily works with adult residents and former residents to develop an individualized plan focused on eliminating barriers and building lives of stability and self-sufficiency in recovery. The Case Manager II provides regular counsel and support for clients setting and progressing on personal goals, accessing services, and guidance for clients struggling with histories of trauma, abuse and domestic violence, substance use disorder, and behavioral health challenges. This role provides SUD services to clients and offers case consultation with peers. The role builds an atmosphere of safety and care in the community home and healthy, supportive engagement with other community members.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Participate in the initial client interviews as agreed upon with manager to determine needed program services and assess the self-sufficiency and safety of potential clients. Objectively assess barriers, needs, and strengths related to housing, financial health, relationships, substance use, and mental health.
Meet weekly with assigned clients to assist with developing goals to achieve stability, including action steps and strategies to achieve success in achieving permanent housing, financial stability, relational health, and life skills.
Track the progress toward the program graduation criteria with clients and make weekly/monthly updates.
Engage using motivational interviewing and trauma-informed care as the foundation for client interaction. When necessary, explain the consequences of actions or inactions as they relate to the achievement of goals.
Support the residents by helping develop an affordable housing plan, which should inform all other goals.
Based on the client plan, refer them to external professional and community services. Administrate and track resource support. Develop parallel plan with partner agencies.
Advocate and broker for clients externally when necessary to achieve goals
Work with clients in financial management, including budgeting and accountability.
Connect clients with the appropriate coaches and classes to support skill development. Advise and coach on best practices.
Work with clients to assess their children for risks academically and developmentally and consult with Kids Program staff.
Explain and enforce program policies and procedures.
Work with clients to encourage forward progress, explaining boundaries and accountability measures through written and verbal communication.
Develop and maintain case files, logs, and other records, including progress reports and periodic evaluations. Complete monthly, operating and statistical reports as required.
Work with program team to develop care plans for the family unit and help execute those plans. Attend and actively participate in weekly staff and case-review meetings as required and accept responsibility for developing positive team relationships.
Participate in Acres community events to create and maintain a caring and fun atmosphere, identify and support residents who may have difficulty interacting, and help them find positive ways to participate safely. This could include community dinners, celebration nights, house meetings, field trips, and representing Acres at community events.
Model healthy relating skills, such as assuming best intentions, initiating respectful conversations regarding conflict, active listening, and asking for feedback.
Maintain strict client and donor confidentiality, objectivity, and professional boundaries.
Establish and maintain good partnerships with agencies serving the Acres of Diamonds community and with agencies referring clients to Acres of Diamonds.
Administer and/or arrange random drug and alcohol tests and ensure complete and up-to-date records.
Attend trainings and conferences both online and in-person to continue developing case management skills, assess industry developments, and to integrate best practices.
Perform other duties as assigned by the Director of Family Services.

The position requires a heart of compassion and love for traumatized families, excellent relational abilities, and the ability to practice sound judgment and discernment.
Education/Experience: Bachelor's degree from four-year college or university and minimum of two years experience providing case management services for those living with substance use disorder; or six or more years related experience and training in Social Work, Counseling, or related Social Services field; or equivalent combination of education and experience. Masters Degree in Social Work (MSW) preferred.
Language Ability: Read and interpret documents in English such as safety rules, operating and maintenance instructions, and procedure manuals. Write routine reports and correspondence. Speak effectively before groups of customers or employees.
Mathematical Ability: Add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Compute rate, ratio and percent, and draw and interpret bar graphs.
Reasoning Ability: Apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Deal with problems involving several concrete variables in standardized situations.
Computer Skills: Proficient in MS Office Suite
Certificates and Licenses: SUDP License in WA State, or equivalent out-of-state certification (per DOH requirements). Valid Washington State driver’s license and ability to travel to frequent offsite meetings.
Ability to pass and maintain acceptable background check per State guidelines.
