Our Pediatric Therapy Team is hiring a Community Based Services Program Coordinator! Join our team of providers and our mission as we partner with families to build healthy foundations and community where all children thrive.

You will be joining a team that is focused on holistic care to meet the critical individual needs of the developing child and support for the family. You will be working with a team of providers offering multidisciplinary care in a community that connects with, encourages, and builds relationships with one another.

Scope: The Community Based Services Program Coordinator provides overall coordination, including community outreach and intake for the Community Based Services program. This role is pivotal in connecting families, schools, and daycare centers with the resources they need by identifying unique needs and streamlining enrollment processes. The Program Coordinator will work closely with the Family Services Manager and Therapy Supervisors to simplify and enhance the client experience, focusing on both external community engagement and internal collaboration.

Community Outreach and Engagement
•Develop and implement outreach strategies to engage with families, schools, daycare centers, and other community stakeholders.
•Organize and participate in community events, workshops, and meetings to promote the Community Based Services program.
•Coordinate developmental screening events and opportunities
•Build and maintain strong relationships with community partners to increase program visibility and access.
•Act as the primary point of contact for community inquiries and referrals regarding the program.
•Create and distribute surveys to families and community partners regarding services rendered. Work with the CBS team to analyze results and make action plans to improve.

Intake and Enrollment Coordination
•Identify the unique needs of families, schools, and daycare centers to tailor the enrollment processes effectively.
•Manage the intake process, ensuring that all necessary documentation is completed accurately and efficiently.
•Coordinate with the Family Services Managers to streamline and enhance the enrollment process.
•Provide support and guidance to clients throughout the enrollment process to ensure a positive experience.



Client Experience Enhancement
•Develop and implement processes and systems to enhance the client experience, making it as seamless and supportive as possible.
•Collect and analyze feedback from clients to continually improve services and address any concerns or challenges.
•Work collaboratively with the Family Services Managers and other team members to create a client-centric approach in service delivery.

Internal Collaboration and Coordination
•Collaborate with Family Services Managers, Program Directors, Managers, and Supervisors to ensure cohesive and integrated service delivery.
•Participate in regular team meetings to discuss program updates, client needs, and collaborative strategies.
•Share insights and best practices with colleagues to foster a culture of continuous improvement and innovation.

Program Administration and Reporting
•Maintain accurate records of all client interactions, enrollment data, and community outreach activities.
•Prepare regular reports on program outcomes, community engagement, and client feedback.
•Ensure compliance with all relevant policies, procedures, regulations and grant requirements.

Qualifications
•Knowledge of child development (ages 0 to 10 years old) and therapeutic supports
•Knowledge of racial equity, determinants of poverty, and social justice frameworks
•Demonstrate a desire to identify, gain knowledge in and awareness of individual experiences and societal oppressions that impact families and children in underserved populations (LGBTQ+, race, gender, socioeconomic, etc)
•Bachelor’s degree in social work, community development, or a related field. Experience may be substituted for degree.
•Proven experience in community outreach, program coordination, or a similar role.
•Strong interpersonal and communication skills, with the ability to engage effectively with diverse community members and stakeholders.
•Excellent organizational skills, with a keen eye for detail and the ability to manage multiple tasks simultaneously.
•Proficiency in Microsoft Office Suite and other relevant software applications.
•Passion for community service and a commitment to enhancing the well-being of families and children.

Preferred Qualifications
•Bilingual or multilingual skills.
•Experience working in family services, education, or a nonprofit organization.
•Knowledge of local community resources and services.

Work Environment
•The Program Coordinator will work in the community, and in an office setting.
•Occasional evening and weekend work may be required to attend community events or meetings.

PHYSICAL DEMANDS:
Work is conducted in a clinical office environment. Incumbent may have some physical requirements associated with working with children and may come into contact with bodily fluids.
